Filled Miscellaneous application form

• 2 recent passport size photographs showing full frontal view of the person's head and shoulders having a white or light-colored background. No glare if wearing glasses.
• Original and self-attested copy of valid Indian Passport (first and last pages)
• Original and self-attested copy of valid Cyprus visa/ residence permit
• Request Letter stating the purpose of Life Certificate
• The issuance of the certificate is subject to verification of the Documents/credentials submitted by the applicant.

SUBMISSION OF APPLICATION FORMS:

Application along with supporting documents may be submitted at the High Commission personally.
